We believe every child deserves the opportunity to grow, heal, and thrive. Our Children's Mental Health Targeted Case Management program (CMH-TCM) is here to support their mental health journey by empowering each child and their family every step of the way.

Our dedicated case manager is skilled at breaking down barriers and building bridges, ensuring that every child has access to a well-coordinated network of care. From creating individualized care plans to connecting families with community resources, support is tailored to each child's unique needs.

    • Work directly with children and their caregivers, collaborating with the ElevaCare treatment team and other key people in the child's life
    • Help set meaningful goals
    • Plan treatment strategies
    • Track progress to support growth, confidence, and emotional well-being

  • Children/adolescents with a severe emotional disturbance (SED) that impacts their daily functioning at school, home, or community. A diagnostic assessment recommending the service is needed.
